Important Instructions

Welcome to online registration.  Before you begin, please read the information to determine residency, tuition amounts and instructions for completing the application.

Prior to completing the online student application, please be prepared to upload the following required information.  The format will not allow you to continue until the requested information is provided.

 

Documents Required for New Students:

  • Copy of Birth Certificate
  • Copy of Social Security card
  • Up to date School Certificate of Immunization on GA Form 3231
  • Vision, Hearing and Dental Certificate on GA Form 3300
  • Copy of transcript (high school) / latest report card (Elem. & Middle school) (not required for Kindergarten)
  • Copy of Parent / Guardian Driver’s License
  • Copy of Custody order (if parents are divorced)
  • Proof of Residency (City of Chickamauga residents only)
    MUST SUBMIT THE FOLLOWING DOCUMENTATION:

    Current City of Chickamauga Utility Bill and Housing Lease/Purchase agreement

Documents Required for Returning Students:

  • Proof of Residency (City of Chickamauga residents only) - Current City of Chickamauga Utility Bill
  • Copy of Custody Order - if there have been changes or updates
